- Appointments are currently available through our reservation system. Please click on the link below to book your appointment.
- If you are simply in need of a passport photo, walk-in service is ONLY available if an agent is free to take it.
- Please arrive for your appointment 15 minutes early with your ID, payments and all required documents completed in BLACK INK ONLY.
- Make sure you reference Travel.State.Gov for all the required documents needed for each individual needing a passport.
- If you have travel plans, please DO NOT list anyone traveling with you as your emergency contact.
- If you are more than 15 minutes late for your appointment, you will need to reschedule.
- Only two applicants per appointment slot. Additional appointments must be reserved for parties of three or more.
- If no appointments show on the calendar, we DO NOT have any openings. Please check back later.

What do I need? (Ages 16+)
For minors ages 16 & 17, at least one parent or guardian must be present to sign and present identification.
- Form must be completed in BLACK INK prior to your appointment. No double-sided form submissions please.
- If you make a mistake, DO NOT attempt to correct it. Complete a new form instead.
- Complete lines 1-22.
- DO NOT SIGN FORM.
Please bring ONE of the following:
- Valid Driver’s License
- Current passport
- Government employee identification
- Recent certification of naturalization

If you were born in the U.S., you must submit ONE of the following:
- Previous U.S. Passport (required if available)
- Certified birth certificate (must contain the names of the parents) Abstract and hospital birth certificates are NOT acceptable

If you were born in a foreign country, you must submit ONE of the following:
- Naturalization Certificate
- Previous U.S. Passport
If you have changed your name from birth, original vital documents such as a marriage and/or divorce must be submitted with your application to reflect that change.
- If you need replacement copies of any vital records, select the vital records request button to connect with the state connected to that life event.

Applicants are responsible for–
- reviewing the checklist of required items
- bringing the required items
- provide required types of payments necessary for their appointments

What do I need? (Ages 15 & under)
For a new or a renewal passport for a child ages 15 and under, the child and both parents, or the child’s legal guardian(s) must be present at the appointment.
- Form must be completed in BLACK INK prior to your appointment. No double-sided form submissions please.

- Complete lines 1-22.
- DO NOT SIGN FORM.
If one parent cannot appear in person, they may need to complete and notarize Form DS-3053.
- All DS-3053 forms must be filled out in BLACK INK
- All DS-3053 forms must include a legible photocopy of the front & back of the non-appearing parent’s valid driver’s license or government-issued identification
Certified birth certificate required (Must contain the names of the parents). Please select the vital records request button if you need additional copies of vital records such as birth certificates and/or custody papers. Abstract and hospital birth certificates are NOT acceptable
Certified Court Order of Guardianship. Request a Certified copy by contacting the court clerk’s office where the guardianship order was issued. Each state has difference requirements for certified copies, so it’s essential to check with them.
