Passport Services @ the Library

BY APPOINTMENT ONLY

• Appointments are currently available through our reservation system – even if you just require a Passport photo.
• Please arrive for your appointment 15 minutes early with your ID, payments and all required documents completed in BLACK INK ONLY. 
• Make sure you reference Travel.State.Gov for all the required documents needed for each individual needing a passport.
• If you are more than 15 minutes late for your appointment, you will need to reschedule.
• Only two applicants per appointment slot. Additional appointments must be reserved for parties of three or more.
• If no appointments show on the calendar, we DO NOT have any openings. Please check back later.
• Traveling in less than 3 weeks? Click here to make an appointment to apply at a passport agency or center.
• Friday afternoon we offer Bilingual Assistance – Viernes por la tarde ofrecemos Servicio Bilingüe.

   

     

What do I need? (Ages 16+)

For minors ages 16 & 17, at least one parent or guardian must be present to sign and present identification.

For a new or a renewal passport for a child ages 15 and under, the child and both parents, or the child’s legal guardian (s) must be present at the appointment.

If one parent cannot appear in person, they may need to complete and notarize Form DS-3053.
