Appointments are currently available through our reservation system.

Please arrive for your appointment 15 minutes early with your ID, payments and all required documents completed in BLACK INK ONLY.

If you are more than 15 minutes late for your appointment, you will need to reschedule.

Only two applicants per appointment slot. Additional appointments must be reserved for parties of three or more.

If no appointments show on the calendar, we DO NOT have any openings. Please check back later.

What do I need? (Ages 16+)

For minors ages 16 & 17, at least one parent or guardian must be present to sign and present identification.

        If you make a mistake, DO NOT attempt to correct it. Complete a new form instead

If you were born in the U.S., you must submit one of the following:

        Certified birth certificate (must contain the names of the parents)

If you were born in a foreign country, you must submit one of the following:

Naturalization Certificate
Previous U.S. Passport

For a new or a renewal passport for a child ages 15 and under, the child and both parents, or the child’s legal guardian (s) must be present at the appointment.

If one parent cannot appear in person, they may need to complete and notarize Form DS-3053.

- - - - All DS-3053 forms must be filled out in BLACK INK
        
        All DS-3053 forms must include a legible photocopy of the front & back of the non-appearing parent’s valid driver’s license or government-issued identification

Abstract and hospital birth certificates are not acceptable
