For a new or a renewal passport for a child ages 15 and under, the child and both parents, or the child’s legal guardian(s) must be present at the appointment.
If one parent cannot appear in person, they may need to complete and notarize Form DS-3053.
Certified birth certificate required (Must contain the names of the parents). Please select the vital records request button if you need additional copies of vital records such as birth certificates and/or custody papers. Abstract and hospital birth certificates are NOT acceptable
Certified Court Order of Guardianship. Request a Certified copy by contacting the court clerk’s office where the guardianship order was issued. Each state has difference requirements for certified copies, so it’s essential to check with them.
